Job Description
As a Telehealth Registered Nurse, you will collaborate with healthcare professionals to deliver personalized care to patients with chronic conditions, autoimmune disorders, oncology diagnoses, and more. Using advanced telehealth technologies, you’ll provide remote education, medication support, and manage care plans for diverse patient populations.
Key Responsibilities:
- Conduct outbound and inbound calls to patients to provide education on disease management, medication adherence, and health maintenance.
- Collaborate with other healthcare professionals to ensure the continuity of care for patients.
- Assist patients and caregivers in understanding treatment plans, medication regimens, and answer health-related questions.
- Track patient interactions, document updates, and provide feedback to improve care outcomes.
- Utilize company-provided telehealth technology for virtual assessments and remote patient monitoring.
Key Requirements:
- Current, unrestricted Registered Nurse (RN) license with compact state privileges or the ability to be licensed in multiple states.
- Proven experience in telehealth, case management, or related fields.
- Exceptional communication skills (both verbal and written) with a focus on patient education.
- Advanced computer skills, including experience using telehealth platforms to engage with patients.
- Experience with chronic disease management, including autoimmune disorders and oncology, is a plus.
